Care and Services
Personalized Care Plans: Customized individualized care designed around each resident’s medical needs, routines, and preferences.
24/7 Nursing & Awake Care Staff: Registered Nurse oversight around the clock, with awake caregivers on-site day and night.
On-Site Nursing & Physician Visits- Regular on-site staffed nursing support and monthly doctor visits through Bluestone Physician Services.
Medication Management & Administration: Safe medication reminders, administration, storage, and ongoing monitoring by trained staff.
Low Caregiver-to-Resident Ratios(1:2–1:5): While the Minnesota average is one caregiver for 18 residents, Heritage of Hope typically serves fewer than three residents per caregiver—allowing for attentive and truly personalized care.
Assistance with Daily Living (ADLs): Support with bathing, dressing, toileting, mobility, and personal hygiene.
Transfers: 1 or 2 personal transfers are available depending on the home.
Clinical Coordination & Monitoring: Nursing coordination of labs, therapy, physician orders, and ongoing clinical documentation.
Specialized Diets & Mealtime Support: Meal preparation, modified diets, and hands-on assistance with eating and drinking when needed.
Mobility & Safety Support: Supervision with walking, transfers, therapy exercises, and fall-prevention care.
Therapy and Other Services Coordination: Our team coordinates regularly with our wider community of physical, speech, social services and more to align with your health goals.
Hospice & Advanced Care Support: Seamless coordination with hospice and comfort-focused care when higher medical support is needed.
Skilled Nursing: We often partner with Skilled nursing for regular wound care
Heritage of Hope serves residents with a wide range of memory and cognitive situations. Our caregivers receive ongoing training to support individuals experiencing memory loss and cognitive changes. Care is always personalized to each resident’s abilities, needs, and safety. We're happy to discuss your situation and to see how we can best serve you.
Dining is one of the most meaningful parts of the day at Heritage of Hope. Meals are often shared around the kitchen table, where conversation flows, laughter is common, and friends naturally become like family.
We provide three thoughtfully prepared meals each day, two snacks, and dessert, all designed to feel familiar, nourishing, and enjoyable. Weekly menus are created with the season in mind and personalized to reflect individual tastes, house favorites, and specific dietary or health needs, with gentle monitoring to support well-being.
Dinner features chef-prepared meals through our partnership with Let’s Dish, offering quality and variety with a restaurant-level touch—while still feeling home-cooked. Breakfast is typically continental and enjoyed on each resident’s schedule, while lunch and dinner are served at set times, always with a main option and an alternative.
Residents may dine at the kitchen table, on the patio, or in the comfort of their room, with flexibility, care, and dignity. The result is dining that feels special without feeling formal—warm, inviting, and deeply life-giving.
Life at Heritage of Hope is designed to be hope-filled, engaging, and shared together. Each home offers a monthly activity calendar with daily options designed to bring joy, connection, and purpose. Activities include games, art, crafts, and meaningful projects that encourage engagement and community.
Throughout the year, our homes come together for seasonal celebrations such as Valentine’s parties, Friends-giving, and a Fourth of July barbecue—sometimes even featuring food trucks. Residents are also invited to monthly sponsored outings, accompanied by our care team to ensure safety and continuity of care. Past outings have included a professional Christmas concert, the Sea Life Aquarium, an apple orchard and more.
Residents are always free to participate as they wish, honoring individual interests, energy, and choice.
Spiritual care is an important and growing part of life at Heritage of Hope. Our story and mission are grounded in Christian values, while honoring and respecting people of all faiths and spiritual journeys.
Residents are supported in practicing their faith in ways that are meaningful to them—whether attending a local church, connecting in conversation with others, participating through an online campus, or engaging privately. In our homes, gatherings often include a simple blessing before meals, with devotional opportunities offered throughout the year. Pastoral prayer is also available for those who desire it. We welcome conversation about how we can best support you spiritually.
Navigating senior care finances can feel overwhelming—we’re here to help. Heritage of Hope serves both private-pay residents and those who may qualify for waiver assistance, and we often support families through the process of exploring and securing available programs.
We work alongside trusted community partners, including senior law and advocacy professionals, to help families understand options, plan wisely, and move forward with confidence. Our team is committed to clear communication, practical guidance, and walking with you every step of the way.
